Application Process
This section includes the steps required to enrol an AISH recipient in the program.

Trust Agreement
The Trust Agreement is the document that enrols the AISH recipient in the program. It is to be completed and signed by the Primary Contact Person and the AISH recipient. This form is required when enrolling an AISH recipient in the program. Below are the PDD Regional Trust Agreement Forms;

Instruction Sheet/Budget (Adobe PDF) Size: 93KB
This form sets out the AISH recipient’s instructions to the Office of the Public Trustee with respect to how their AISH benefits should be spent (i.e. it is the AISH recipient’s budget.) This form is required when enrolling an AISH recipient in the program.

How to Make Changes to the Instruction Sheet/Budget (Adobe PDF) Size: 60KB
If a participant wishes to change, add or delete an instruction, a new instruction sheet must be completed and submitted to the Office of the Public Trustee. This section explains how to do this.

Contact Verification Form (Adobe PDF) Size: 60KB
This is an attachment to the Trust Agreement and is to be signed by the Program Contact Person. This form is required when enrolling an AISH recipient in the program.

Acknowledgement Form (Adobe PDF) Size: 78KB
This is an attachment to the Trust Agreement and is to be signed by the AISH Financial Benefits Worker or AISH Administrator. This form is required when enrolling an AISH recipient in the program.

Termination Notice (Adobe PDF) Size: 60KB
This is an attachment to the Trust Agreement. If the AISH recipient wishes to terminate their involvement in the program, this form must be signed by them and given to their Primary Contact Person.
