Pilot Site Evaluations

The Person Centered Planning Pilot project in Central included having pilot sites complete two sets of evaluation questions.

The first set focused on the actual outcomes achieved and impacts related to individuals, families, agencies and local communities.

Pilot Site Overall Evaluation (PDF document)

The second set of evaluation questions was intended to collect feedback on what activities or actions were undertaken within the pilot agencies that seemed to be helpful in successful implementing Person Centered Planning during the Pilot phase.
