you are eligible, a community support coordinator
will schedule a home visit to meet with you and
your support network. The purpose of the meeting is to determine
the
services
and
supports requested and to develop an action plan.
What will Community Support Coordinators
do for You?
- Work with you and your
circle of support to clarify short and
long-term plans
- Provide information about options such as
community living, employment, and
specialized services such as behavioural and
emergency supports, and transportation
- Assist with building a support network
- Provide information on funding options
- Support you when changes occur
- Maintain contact to ensure current
supports are responding to your
expectations
- Facilitate problem solving if there are
difficulties
- Prepare plans, budgets and applications
for funding approval
- Administer the individual funding program
Additional Considerations
Income support for adults with developmental
disabilities can be accessed through the Assured
Income for the Severely Handicapped
Program (AISH) or the Supports for Independence
Program (SFI).

are
available at the Office
of the Public Guardian at 427-0017, or
at the Office
of the Public Trustee at 427-2744. "How
to" workshops are regularly provided by
the Society for the Retired and Semi Retired.
