2008 Guardian Satisfaction Survey

Every two years, PDD asks Guardians to assess their level of satisfaction with the PDD-funded services available to your family member or the person whom you represent. This survey provides important information for the Boards in assessing performance and identifying areas for improvement. Board Meeting

The 2008 survey has been sent to all Guardians who now have two options for responding.

• The paper survey can be completed and returned to the Vocational Rehabilitation and Research Institute (VRRI) using the stamped envelope included in the survey package, OR,
• An ‘on-line' form can be completed and submitted.

Instructions for completing the survey using either method are included in the package mailed out to Guardians. Anyone using the ‘on line' option, should NOT return the paper survey.

All survey responses are anonymous and Guardians will not be personally identified in any way.

VRRI will analyze the survey results independently and prepare a report for the boards.

Guardians are asked to have the completed surveys returned by October 10, 2008.
