My Life Workbook

During 2002-2003, individuals with developmental disabilities in Central Alberta were engaged in the development of a monitoring and accountability tool as part of their work in the "Broadening Your Horizons" project.

The Monitoring and Accountability Tool became known as the My Life Workbook.

The My Life Workbook provides an opportunity for individuals with developmental disabilities to assess their circumstances and use the resulting information to change or adjust their goals or their supports and services. 

The My Life Workbook is based on the 12 Quality of Life standards that are part of the Creating Excellence Together certification process operated by the Alberta Association of Rehabilitation Centers (AARC).

Individuals with developmental disabilities, who are interested in using the My Life Workbook, are invited to call a MENTOR to get some help. Mentors are self-advocates who have volunteered to be available to help others in completing the My Life Workbook.
